ABSTRACT

OBJETIVE To identify the recurrence rate of giant-cell tumor (GCT) in patients treated by curetage and adjuvant therapy with polymethil metacrylate with a minimum followup of 3 years. MATERIALS AND METHODS Observational and retrospective cohort with patients with diagnoses of Enneking stages 1 and 2 GCT treated through intralesional curetage and cementation between 1981 and 2011. Age, gender, anatomic location and relapse period were recorded. The data was analyzed with measures of central tendency and dispersion (standard deviation) for the quantitative variables, percentages for the qualitative variables. RESULTS Between 1981 and 2011 375 cases of GCT were identified, 141 (36.7%) of which were treated by this method. The follow-up ranged from 48 to 240 months, the age was of 27 9 years, 45% of the patients were female, and 55%, male, with a female: male ratio of about 1.2:1. The tibia was the most frequent affected bone (38%), followed by the femur (32%), the humerus (16%), and the radius (10%). The resection thecnique was intralesional curetage in about 88.6% of the cases, and marginal resection in the remaining cases. Pathologic fracture was present in approximately 15.7%, and recurrence occurred in approximately 12.7%. DISCUSSION We demonstrated that this treatment method decreases the risk of recurrence due to the local adjuvant effects of acrylic cementation. Recurrence events occur in the first two years after resection. However, some authors defend that the surgical margin is the only factor that influences the risk of local recurrence. The extraosseus extension of GCT is not a contraindication to perform intralesional curetage or to prescribe the adjuvant treatment with polymethyl metacrylate. CONCLUSIONS We reported a recurrence rate similar to that of the literature, and this is a feasible resource for limb reconstruction

ABSTRACT

Abstract Introduction Adenoidectomy can be performed with many ways, including curettage and microdebrider endoscopic-assisted adenoidectomy. Those two techniques have advantages and disadvantages. Objective The objective of this study is to research the effects of curettage adenoidectomy and endoscopic-assisted microdebrider adenoidectomy on the tympanum pressures in pediatric patients with adenoid hypertrophy without otitis media with effusion. Methods This prospective descriptive study was performed with 65 patients who had a normal tympanic membrane and normal tympanogram and then underwent adenoidectomy or adenotonsillectomy for adenoid and tonsil hypertrophy. The subjects were randomly divided into two groups: curettage adenoidectomy group and endoscopic microdebrider-assisted adenoidectomy group. They underwent tympanometry, and the preoperative as well as 1st and 7th day postoperative values of the tympanum pressures were compared within and among the groups. Results There were 32 patients in the curettage adenoidectomy group and 33 patients in the microdebrider adenoidectomy group. Statistically significant differences were observed in the median tympanum pressure on the preoperative and 1st and 7th postoperative days for both the left and right ears with curettage adenoidectomy (p < 0.001, p < 0.001). This difference occurred on the 1st postoperative day, and the value returned to normal on the 7th day. There was no significant difference in the median tympanum pressure on the preoperative and 1st and 7th postoperative days for both the left and right ears in the microdebrider adenoidectomy group (p = 0.376, p = 0.128). Conclusion Postoperative Eustachian tube dysfunction is seen less often with the endoscopic-assisted microdebrider adenoidectomy technique than with the conventional adenoidectomy technique.

ABSTRACT

ABSTRACT A history of C-sections is the most important risk factor for the development of placenta accreta spectrum disorders. The global elevation of caesarean section indicator has led to increase the incidence of these disorders. However, other antenatal conditions should be considered among the risks, associated with less extensive surgical damage of the uterine wall, such as curettage. A literature review on the subject of placental accreta spectrum disorders is carried out to deepen on the serious impact on pregnancy. Screening of placenta accreta is recommended in all pregnant women with a history of uterine curettage, as part of prenatal care. Placenta accreta is generally an entirely iatrogenic condition. Health professionals and patients could contribute to the reduction of this condition incidence, in regards to abortion(AU)

ABSTRACT

Vitiligo is a dermatosis requiring complex treatment. In clinically stable cases, melanocyte autologous grafting has shown good results, using different methodologies for obtaining the donor area: dermatomes, punches, blisters. However, these techniques are complex and require specific instruments. This study presents a simple technique for melanocyte harvest that has been performed in our service for more than ten years and includes epidermal curettage of the donor area, grafting of the obtained material, which was homogenized with saline or hyaluronic acid, on the achromic area, and the application of a semipermeable dressing that is removed 1 week later.

ABSTRACT

BACKGROUND: This study was conducted to examine the clinical usefulness and efficacy of endoscopic curettage on benign bone tumor. METHODS: Thirty-two patients (20 men and 12 women) with benign bone tumor were included in the study. The patients were aged between five and 76 years; the mean follow-up period was 27.05 months (range, 9.6 to 39.9 months). The primary sites include simple bone cyst (9 cases), fibrous dysplasia (6 cases), enchondroma (5 cases), non-ossifying fibroma (4 cases), bone infarct (3 cases), aneurysmal bone cyst (1 case), chondroblastoma (1 case), osteoblastoma (1 case), intraosseous lipoma (1 case), and Brodie abscess (1 case). A plain radiography was performed to assess the radiological recovery. Radiological outcomes, including local recurrence and bone union, were evaluated as excellent, good, poor, and recurred. RESULTS: In our series, there were 27 cases (84.4%) of good or better outcomes, six cases (18.8%) of complications (4 local recurrence, 1 wound infection, and 1 pathologic fracture). CONCLUSIONS: Our results showed that endoscopic curettage and bone graft had a lower rate of recurrence and a higher cure rate in cases of benign bone tumor. It can, therefore, be concluded that endoscopic curettage and bone graft might be good treatment modalities for benign bone tumors.

ABSTRACT

BACKGROUND: Infective spondylodiscitis usually occurs in patients of older age, immunocompromisation, co-morbidity, and individuals suffering from an overall poor general condition unable to undergo reconstructive anterior and posterior surgeries. Therefore, an alternative, less aggressive surgical method is needed for these select cases of infective spondylodiscitis. This retrospective clinical case series reports our novel surgical technique for the treatment of infective spondylodiscitis. METHODS: Between January 2005 and July 2011, among 48 patients who were diagnosed with pyogenic lumbar spondylodiscitis or tuberculosis lumbar spondylodiscitis, 10 patients (7 males and 3 females; 68 years and 48 to 78 years, respectively) underwent transpedicular curettage and drainage. The mean postoperative follow-up period was 29 months (range, 7 to 61 months). The pedicle screws were inserted to the adjacent healthy vertebrae in the usual manner. After insertion of pedicle screws, the drainage pedicle holes were made through pedicles of infected vertebra(e) in order to prevent possible seeding of infective emboli to the healthy vertebra, as the same instruments and utensils are used for both pedicle screws and the drainage holes. A minimum of 15,000 mL of sterilized normal saline was used for continuous irrigation through the pedicular pathways until the drained fluid looked clear. RESULTS: All patients' symptoms and inflammatory markers significantly improved clinically between postoperative 2 weeks and postoperative 3 months, and they were satisfied with their clinical results. Radiologically, all patients reached the spontaneous fusion between infected vertebrae and 3 patients had the screw pulled-out but they were clinically tolerable. CONCLUSIONS: We suggest that our method of transpedicular curettage and drainage is a useful technique in regards to the treatment of infectious spondylodiscitic patients, who could not tolerate conventional combined anterior and posterior surgery due to multiple co-morbidities, multiple level infectious lesions and poor general condition.

ABSTRACT

Some services in Brazil combine curettage of the nail matrix with phenolization in the treatment of ingrown nails, with the objective of further increasing efficacy. The objective of this research was to compare the cure rates between the phenolization technique and phenolization associated with nail matrix curettage. A retrospective study was done which included 271 cases. There was only a 5 percent recurrence rate for the phenolization procedure, compared with 7.7 percent for combined phenolization/nail matrix curettage. There was no statistically significant difference between the two techniques, which indicated that there is no need for curettage of the nail matrix. Phenolization worked even for level III disease.

ABSTRACT

OBJECTIVES: To evaluate the efficacy of cauterization as a sole adjuvant method inintralesional resection of benign bone tumors, taking into account associated factors that could influence local recurrence. METHODS: 47 patients diagnosed with different benign bone tumors were included in this study, all of whom had been treated with curettage associated with cauterization as an adjuvant method. The medium follow-up was 32.7 ± 19.4 months. The recurrence rate was then evaluated in relation to age, sex, location of the tumor, histologic type, tumor grade ( B1, B2 or B3 ), and size and type of material used to fill the tumor cavity ( PMMA or autologus bone). The type and percentage of complications were also included in the analysis. RESULTS: Global recurrence was 19.1 percent. There was statistic significance relating tumor grade to time to recurrence (p=0,028). No other factors were found that directly correlated with global recurrence of the tumors. CONCLUSION: Cauterization proved to be a simple, inexpensive and effective adjuvant method for the treatment of benign bone tumors. Level of Evidence: Level IV, case series.

ABSTRACT

OBJECTIVE: To determine agreement of preoperative and postoperative histopathology of endometrial hyperplasia (EH). MATERIALS AND METHODS: Histopathology of specimens obtained by curettage and hysterectomy within 1 year was retrospectively compared by a skilled gynecological pathologist. Patients who received hormone therapy were excluded. RESULTS: Of 79 women with a preoperative diagnosis of EH, only 32 were diagnosed as EH from hysterectomy specimens. There was no endometrial cancer. The agreement between preoperative and postoperative histology did not achieve statistical significance (Kappa 0.011). Postoperative histopathology was more severe than preoperative diagnosis in 5 (6.3%) patients, including 3 preoperative diagnoses of simple hyperplasia without atypia, 1 simple hyperplasia with atypia, and 1 complex hyperplasia without atypia. CONCLUSIONS: For EH diagnosed by curettage, we can be sure of the diagnosis. However, 6.3% had more severe histology from hysterectomy specimens. Thus, repeated curettage or other investigations should be reconsidered in women with recurrent bleeding.

ABSTRACT

ABSTRACT

A review of 20 patients with aneurysmal bone cyst involving different anatomic locations in the skeleton who were evaluated and staged according to Enneking et al., [1-5] system as 11 active benign lesions [55%] and 9 aggressive benign lesions [45%]. Meticulous curettage was achieved in 15 patients and autogenous bone grafting was applied in 11 patients, 3 of them paternal bone grafting was used and in 4 cases no grafts was applied. In 5 patients resection of the affected bony tubular segment was achieved without reconstruction in 4 of them and a fibular grafting done for the fifth. The mean age at surgery was 16 years old. Union was achieved in all the cases at an average of 9 months. As regard the complications that needs reoperations was in 4 cases of local recurrence and graft resorption necessitating re-grafting, 2 cases with nerve palsy spontaneously improved during the follow up and one case with residual coxa vara which corrected after cyst healing. The mean follow up period was 22.4 months. The functional results of the procedure were rated as excellent in 18 cases [90%], fair in one case [5%], poor in one case [5%]. The end results of the procedure are justifiable for tumor surgery provides cyst healing and ossification with sufficient biological strength and durability with a stable limb and excellent functional end result

ABSTRACT

To evaluate the frequency of clinico-radiological presentation and the prognosis after treatment of chondroblastoma. Design: A descriptive, observational study. Place and Duration of Study :The study was carried out at the Department of Orthopaedic Surgery, JPMC, Karachi over the period of 16 years, January 1986 to June 2002. Methodology: Patients in teenage, presenting with signs and symptoms of a primary bone tumours, were included by retrospectively reviewing the tumour registry record. Detailed scrutiny of clinical record was carried out to analyze the clinico radiological presentation and posttreatment prognosis criteria of healing was symptom-free centripetal and homogenous ossification. Seven [1.24%] out of 563 primary bone tumour cases were chondroblastoma. The clinico-radiological presentation in the 2 out of 7 [28.57%] cases was classical, 2 simulated osteomyelitis and other 3 as chondromyxoid fibroma, aneurysmal bone cyst and giant cell tumour. Lesions were treated with curettage and bone grafting. One patient [14.28%] developed recurrence within 2 years after curettage and healed after re-curettage and bone grafting. Five out of 7 [71,42%] tumours showed good response and 2 out of 7[28.57%] showed satisfactory response. Chondroblastoma is very rare benign primary bone tumour with high local recurrence rate. The lesions confined to epiphysis of weight-bearing bone present early with symptoms of mild arthritis. Lesions in non-weight -bearing bones often present late and simulates epiphysio-metaphyseal tumours. Lesions heals with centripetal healing and need more than 2 years follow-up to achieve recurrence-free healing
